YCSS provides complex case management support to children, young people and families in need.
YCSS case management is strengths-based and a collaborative approach to working with children, young people and families with complex needs.
Case management is delivered through a high level of coordination, key professional relationships and direct work with external services in line with the individual’s needs. YCSS offers evidence-based outcomes for children, young people and families and are committed to delivering a high standard of service in line with the HSQF Licensing Standards.
Therapeutic Residential Care Program:
YCSS provides therapeutic residential care to young people including:
• Emergency and short-term placements;
• Respite placements;
• Long term placements.
Each care model is tailored to the individual needs and stage of their care journey.
Within the YCSS residential care services, young people receive individualised intensive therapeutic support through a trauma-informed and holistic approach.
YCSS also offers a range of counselling and therapy programs including:
• Solution-focused counselling
Keeping Kids at Home (KKAT) Program
The KKAT program has been developed to support children and young people who are on the edge of care. We use prevention and early intervention frameworks to avoid family breakdown. KKAT also works with young people transitioning from out of home care and back into the family home.
The KKAT program provides intensive in-home support aimed at:
• Providing tools and strategies to educate and empower parents and ensure their children are thriving in their environment
• Preserve families and provide holistic wrap around supports
• Assist children and young people to build resilience and employ a strengths-based approach
